Questionnaire Requests
The cost of a questionnaire is determined by turnaround time.
- Non-Rush=7 day turnaround=$150.00
- Rush=4 day turnaround=$200.00
Checks and credit cards are accepted forms of payment.
Payment Instructions:
- Check: Checks can be made out to East West Urban Management, LLC and sent to 1610 Little Raven Street #125, Denver, CO 80202. Please note the building and Unit number in the check memo and include the questionnaire to be completed.
- Credit Card: www.smartstreet.com, make payments, make one-time payment as guest, type “East West” in the association box and click “find”, select East West Urban Management Work Order Account. In the second box, enter the homeowner ID. The homeowner ID is the building name and Unit number (example: Tower 900). Enter payment amount and select pay by credit/debit card or by e-check. There is a $14.95 convince fee charged by the bank when paying with credit card.
After submitting your credit card payment, forward your confirmation email to questionnaire@eastwestum.com. Please include your blank custom questionnaire form or a standard form will be provided.
